What is a Patient-Centered Medical Home?

“Patient-centered” means that you, our patient, are the focus of everything we do. “Medical home” describes what we want to be for you – a place to go for help with any health issues you face. Caring about you is your teams’ number one job. The care your medical home offers is all about you and your health needs.

How does it work?

You have easy access to care:

  • Our staff can be reached around the clock
  • You can get in touch with us by phone or through our secure patient website, MyChart
  • You can schedule same day appointments

We’ll take the time to get to know you by:

  • Learning about you and your health
  • Listening to you and what’s important
  • Talking about your health care choices with you
  • Helping you understand your health conditions
  • Offering you tools to help you take care of yourself

Your health care team works together and will:

  • Get you help with specialist appointments
  • Talk to others involved in your care
  • Share test results with you
  • Access the largest collection of skilled providers in Maine
  • Make a smooth transition between everyone involved in your care

How to work with your team

It’s important that you feel you can:

  • Provide us with your complete medical history, including a list of your medicines and information about care you have received outside of Maine Medical Partners
  • Transfer your medical records to us
